Residency Checklist
Players are eligible to play with that league only if they reside, or the physical location of the school where they attend classes is, within the boundaries provided to, and approved by, Little League® International.  Residence should be established by providing documentation outlined on the Residency and School Attendance Eligibility Checklist.

School Enrollment Form
School attendance shall be established and supported by providing the Little League School Enrollment Form.

Medical Release Form
A medical release is required to be signed by each players parent(s) or legal guardian. The form discloses information about allergies and other maladies that may impact a child's health.
